Amy Brown, RN, BSN Director of Case Management			Physical therapy Increase, restore or maintain range of motion, strength, flexibility, coordination, balance or endurance after an injury, illness or surgery. Occupational therapy Improving daily living skills including bathing, dressing and cooking to achieve an independent and satisfying lifestyle. Speech therapy Identify and treat swallowing, speech and language disorders caused by disease or illness.		REHABILITATION
							Hope Creek offers transitional care to patients following a surgical procedure or other hospitalization. A specialized program of therapies is designed for each patient to restore functionality. Skills learned in rehabilitation establish a foundation for continual progress once the patient returns home.		Physical, Occupational and Speech therapies are offered in-house and covered under Medicare Part A or B, private insurance and Medicaid. Restorative nursing, occupational and physical rehabilitation programs are offered as well.
